There are a million pieces to it, managing a business. There are so many things that cannot be forgotten about or brushed aside and need simultaneous attention. The buck stops at the top and leaders know that ultimately, no matter who owns the error, the glory and the pain eventually belong to them and them alone. This can make delegation difficult.
